Focal and segmental glomerulosclerosis with dissociation of clinical and pathological predictors in the regional hospital of Ica, Peru

The most common clinical presentation of Focal and Segmental Glomerulosclerosis (FSGS) is nephrotic syndrome, microscopic hematuria, and in a significant percentage, high blood pressure and renal failure. We present the case of a 19-year-old patient, with no relevant history, who had been ill for two months, presenting with edema of the lower limbs and face, decreased urinary volume, and acute renal failure. Physical examination revealed palpebral and leg edema. Auxiliary tests showed elevated creatinine and urea, urine test with cylinders, abnormal proteinuria, negative ANA, Anti-DNA, hepatitis, and HIV, normal C3 and C4 levels, and normal renal ultrasound. Renal biopsy showed a pattern consistent with FSGS.
