Introduction: Situs inversus totalis is a rare congenital anomaly characterized by a mirror-image arrangement of the thoracic and abdominal organs. Although it is usually asymptomatic, its presence may hinder the timely diagnosis of acute abdominal conditions due to an atypical clinical presentation.
Case presentation: We report the case of an adult patient who presented to the emergency department with acute abdominal pain of an unusual location. During the diagnostic workup and surgical management, a previously undiagnosed situs inversus totalis was identified, associated with acute appendicitis.
Conclusion: Situs inversus totalis represents a significant diagnostic challenge in the setting of acute abdomen. Clinical suspicion, together with the timely and appropriate use of imaging techniques, is essential to avoid therapeutic delays and prevent complications.
